A Health Home (aka Medicaid Health Home) — as defined in Section 2703 of the Affordable Care Act — offers care coordination to individuals living with multiple chronic health conditions, including HIV, Mental Health, Substance Use Disorders, Diabetes, Hypertension and Asthma. Uptown Healthcare Management is a Care Management Agency within the Health Home network entrusted with providing care coordination to these individuals.
